No major tests are due specifically at week 22, but keep these on your radar:

Coming Up Soon (24-28 Weeks)

  • GDM screening (DIPSI method) — 75g oral glucose, blood sugar checked at 2 hours. A value >= 140 mg/dL = gestational diabetes. GDM prevalence in India is 9.9-21.8%, so this test is critical
  • Anomaly scan — if not already done between weeks 18-22, get it done now. This detailed ultrasound checks all major organs, spine, and placenta position

Ongoing

  • Regular blood pressure monitoring at every visit — preeclampsia can develop after 20 weeks (affects 8-10% of pregnancies in India)
  • Urine protein check at each visit

Nutrition This Week

Second trimester: you need about 350 extra calories per day (ICMR guideline). That’s roughly an extra roti with dal, or a bowl of curd with fruit.

What to Prioritize

  • Calcium: 1000 mg/day — doodh, dahi, paneer, ragi, sesame seeds (til). Baby’s bones are hardening
  • Iron: 60 mg/day — palak, beetroot, pomegranate, jaggery, plus your supplement
  • DHA/Omega-3 — important for baby’s brain development. Fish (rohu, pomfret), walnuts, flaxseeds
  • Fiber — constipation is common. Whole grains, fruits with skin, sabzi, isabgol if needed
  • Protein — 78g/day (ICMR). Dal, paneer, eggs, chicken, chana, rajma

What to Avoid

  • Excess salt — can worsen swelling and blood pressure
  • Maida-heavy foods — low nutrition, worsens constipation
  • Raw or undercooked eggs/meat
  • Unpasteurized dairy
  • Caffeine — keep under 200 mg/day

When to Call Your Doctor

Watch for these warning signs — some are especially relevant from week 20 onward:

  • Severe headache that doesn’t go away — could be preeclampsia
  • Visual disturbances — blurred vision, seeing spots or flashing lights
  • Sudden swelling of face, hands, or feet (gradual foot swelling is normal)
  • Vaginal bleeding — any amount
  • Fluid gushing or leaking from vagina — could be PPROM (preterm premature rupture of membranes)
  • Regular contractions — more than 4-6 per hour at this stage is not normal
  • Severe abdominal pain
  • Reduced fetal movements — if baby was moving and suddenly stops
