Baby Development at Week 24

Your baby is growing fast and gaining weight:

  • Lungs are developing — branching of airways is happening, but they’re not yet mature enough for breathing. Surfactant (the substance that keeps air sacs open) production begins around week 26
  • Weighs about 600g — steadily adding fat each day
  • Skin is thin but less transparent — developing layers
  • Inner ear is maturing — baby’s sense of balance is developing. They know which way is up and down
  • Skin is becoming less translucent — fat deposits are increasing under the skin, giving baby a less see-through appearance
  • Taste buds are functional — baby can taste the amniotic fluid, which changes flavor based on what you eat

If baby were born now, survival rates with intensive NICU care are around 40-60% depending on NICU facility. Each additional week dramatically improves outcomes.

GDM Screening — Do It Now

This is the most important test between weeks 24-28:

  • DIPSI method (Diabetes in Pregnancy Study Group of India):
    • Drink 75g glucose solution (non-fasting is acceptable)
    • Blood sugar checked at 2 hours
    • Positive if >= 140 mg/dL
  • Why it matters: GDM prevalence in India is 9.9-21.8%, among the highest globally. Risk factors include family history of diabetes, PCOS, obesity, and age > 25
  • If positive: Diet modification is the first step. About 80% of GDM cases are managed with diet alone. Insulin may be needed if diet doesn’t achieve targets
  • Blood sugar targets with GDM: Fasting < 95 mg/dL, 2-hour post-meal < 120 mg/dL

Routine Checks

  • Blood pressure — every visit
  • Urine for protein and glucose — every visit
  • Weight monitoring
  • Fundal height measurement

Nutrition This Week

With GDM screening due, this is a good time to evaluate your carb intake — not just for the test, but for overall pregnancy health.

What to Prioritize

  • Complex carbs over simple carbs — roti over white bread, brown rice over polished rice, oats, jowar, bajra
  • Protein with every meal — helps stabilize blood sugar. Dal, paneer, eggs, curd, chicken
  • Calcium: 1000 mg/day — doodh, dahi, ragi, til, green leafy vegetables
  • Iron — continue supplements. Pair with vitamin C foods
  • Fiber — whole grains, vegetables, fruits with skin. Helps constipation and blood sugar control
  • Healthy fats — ghee (moderate), walnuts, flaxseeds, fish

What to Avoid

  • Refined sugar, mithai, sugary drinks — especially if GDM is a concern
  • Maida — naan, white bread, biscuits, cakes
  • Very large meals — eat smaller portions more frequently
  • Excessive fruit juice — whole fruits are better (fiber slows sugar absorption)

When to Call Your Doctor

These symptoms need immediate attention:

  • Severe headache that won’t go away with rest — preeclampsia warning
  • Visual disturbances — blurred vision, flashing lights, seeing spots
  • Sudden swelling of face or hands — not the gradual foot swelling
  • Vaginal bleeding — any amount
  • Fluid leaking or gushing from vagina — PPROM
  • Regular, painful contractions — preterm labor
  • Severe upper abdominal pain (especially right side) — could be HELLP syndrome
  • Significantly reduced fetal movement — baby was active and suddenly quiet
  • Painful urination with fever — UTI needs treatment to prevent preterm labor
